Payment and Insurance Information

Thank you for choosing the Arizona Center for Chronic Pelvic Pain for your care. I wanted to take a moment to explain our payment structure and help you understand your options.
Why We're a Direct-Pay Practice
Our practice operates on a direct-pay model, which means we don't work directly with insurance companies. Here's why: many of the specialized treatments we provide—particularly for conditions like pudendal neuralgia, pudendal nerve entrapment, pelvic floor muscle spasm, and neuropathic pelvic pain—receive little to no coverage from insurance companies.
To continue offering these specialized treatments that you need, we had to make the difficult decision to operate outside the insurance system entirely. This allows us to focus completely on providing you with the most effective care possible, rather than being limited by what insurance companies are willing to cover.
Good News About Surgery Coverage
While we understand that medical expenses can be a concern, we have some encouraging news about surgical procedures. We've partnered with MD Capital, a specialized billing company in New York, to help submit insurance claims on your behalf for major surgeries.
What this means for you:
- We can often help you get reimbursed for procedures like pudendal nerve decompression surgery, endometriosis resection, and other major surgeries
- This works with most commercial insurance plans (though unfortunately not with Medicare alone)
- The process takes 6-9 months, but we are anticipating good success with reimbursements
- This covers the surgeon's fees (our professional fees)
Important Notes:
- Hospital fees are separate—HonorHealth Shea Hospital and Piper Surgery Center handle their own billing
- Out-of-state patients with out of state coverage generally have their hospital fees covered
- Treatments like Botox, Daxxify, and Exosome therapy are rarely covered by insurance
Our Commitment to You
While we can't guarantee insurance reimbursement, our experience shows that many patients with commercial insurance should receive significant or complete reimbursements for the professional part (surgeon’s fee) of the procedure. We're committed to helping you navigate this process and will work with you every step of the way.
